Age Well What is Age-Friendly Health Care?
The Institute for Healthcare Improvement (IHI) has designated the AdventHealth Medical Group locations across Western North Carolina as Age-Friendly Health System – Committed to Care Excellence. As an older adult, you can count on your AdventHealth provider to focus on the four essentials of care as identified by the Age-Friendly Health Systems Initiative. Your physician will learn about your goals and preferences within the framework below at your annual wellness visit each year, and your care team will work with you based on what matters to you.

When you attend your annual wellness visit each year, your internal medicine provider will ask you a question:
What matters most to you?
Whether it’s spending time with family, volunteering at your favorite non-profit, enjoying time in nature, or caring for four-legged friends, our providers work with you to design an individual health care plan unique to your specific goals and preferences so you can live the life that keeps you feeling whole.

Your internal medicine provider will work with you to make sure that when necessary, medications are selected that do not interfere with the things that matter most to you, your mentation (ability to think clearly), or your mobility.

As you age, you may experience changes in the way you think and feel. Your internal medicine provider will work with you and your caregiver to identify, treat, and manage dementia, depression, and delirium across all settings of care including AdventHealth specialists and hospital services.

As an older adult you may begin experiencing changes in your ability to move. Your internal medicine provider will work with you to ensure you have the tools needed to move about safely and continue doing the things that matter most to you.

Internal Medicine
AdventHealth internal medicine physicians practice in a variety of locations in Henderson and Buncombe counties and welcome patients 18 years and older with a focus on older adults. Internal medicine providers have additional training on more complex health conditions. Older adults can benefit from working with an internal medicine provider for close medication monitoring as well as management of multiple specialists.

Family Medicine
Family medicine providers are ideal for people who are generally in good health and want continuous care to stay well. Our family medicine physicians practice in a variety of locations in Henderson, Buncombe, Transylvania and Polk counties, and welcome patients of all ages.

Emergency Care
AdventHealth Hendersonville is proud to have earned the Geriatric Emergency Department accreditation awarded by the American College of Emergency Physicians to emergency departments that have developed a comprehensive approach to caring for older patients.
